Russia poised for largest naval exercise for decades

MOSCOW (Reuters) - Russian warships have embarked on a long voyage to the Black and Mediterranean seas to take part in what the Defence Ministry said would be the largest naval exercise in decades.

It said on Wednesday that ships from its Northern, Baltic, Black Sea and Pacific fleets would stage the exercise at the end of the month to test their ability to act together outside Russian waters.

Its website said the training exercise would also include anti-terrorism and anti-piracy drills.

"A Navy exercise on such a scale is being staged for the first time in recent decades," the ministry said, without giving other details such as how many ships would take part.

Russia regularly stages naval war games involving different fleets, and in August sent ships to the Mediterranean for a combined training exercise.

State-owned RIA Novosti news agency said that that exercise had involved three large amphibious assault ships, two frigates, a destroyer and two support ships.

Moscow has been trying to strengthen its military presence in the Mediterranean region.

President Vladimir Putin, a former operative for the Soviet Union's KGB national security agency, says Russia needs a stronger army to protect it from foreign attempts to stoke conflicts around its borders.

Russia plans to spend 23 trillion roubles ($753 billion) over a decade to modernize the former superpower's armed forces, which underwent a decade of spending cuts after the fall of the Soviet Union in 1991.

The Defence Ministry did not say if the coming deployment was connected to the conflict in Syria. Moscow has been a staunch supporter of President Bashar al-Assad and his largest arms supplier.

Last month, a naval source told Interfax news agency that Russia was sending warships to the Mediterranean in case it needed to evacuate citizens trapped by the civil war in Syria.

Also in December, Itar-Tass and Interfax cited military sources as saying two landing craft had left a Black Sea port and would call at Russia's naval supply and maintenance facility in the Syrian port of Tartous.

BANJUL (Reuters) - AIDS patients would be offered an herbal cure at a 1,111-bed hospital in Gambia that the president said on Tuesday he plans to build despite medical concerns the treatment is dangerous.

President Yahya Jammeh said in 2007 he had found a remedy of boiled herbs to cure AIDS, stirring anger among Western medical experts who claimed he was giving false hope to the sick.

"With this project coming to fruition, we intend to treat 10,000 HIV/AIDS patients every six months through natural medicine," Jammeh said in his New Year's address, adding that he expected the 1,111-bed hospital to open in 2015.

The World Health Organisation and the United Nations have said Jammeh's HIV/AIDS treatment is alarming mainly because patients are required to cease their anti-retroviral drugs, making them more prone to infection.

Jammeh said in October that 68 HIV/AIDS patients undergoing his herbal remedy had been cured and discharged, the seventh batch since the treatments began five years ago.

Other African leaders have drawn criticism for extolling the power of natural remedies to combat AIDS.

The administration of former South African President Thabo Mbeki was ridiculed for denying there was a link between HIV and AIDS while prescribing meaningless treatments such as beet root instead of internationally proven medicines.

The HIV rate in Gambia is relatively low compared to other African states, with 2 percent of the country's roughly 1.8 million people infected, according to the United Nations.

Differences in generic pill characteristics may lead to interruptions in essential medication use

Dec. 31, 2012 ? Generic medications currently account for over 70 percent of prescriptions dispensed. However, while generic drugs are clinically bioequivalent to the brand-name version, they often differ in their physical characteristics, such as color and shape. Researchers from Brigham and Women's Hospital (BWH) have found that some patients who receive generic drugs that vary in their color are over 50 percent more likely to stop taking the drug, leading to potentially important and potentially adverse clinical effects.

The study will be published online Dec. 31, 2012 in the Archives of Internal Medicine.

"Pill appearance has long been suspected to be linked to medication adherence, yet this is the first empirical analysis that we know of that directly links pills' physical characteristics to patients' adherence behavior," explained Aaron S. Kesselheim MD, JD, MPH, assistant professor of medicine in the Division of Pharmacoepidemiology and Pharmacoeconomics at BWH, and principal investigator of this study. "We found that changes in pill color significantly increase the odds that patients will stop taking their drugs as prescribed."

The researchers conducted a case-control study of patients taking antiepileptic drugs and compared the odds that patients who did not refill their medication had been given pills that differed in color or shape from the prior prescriptions. Using a large national database of filled prescriptions, when the researchers identified a break in the patient's use of the drug, they looked at the previous two prescription fillings to see if they were the same color and shape. They found that interruptions in the prescription filling occurred significantly more frequently when the pills had different color. Interruptions in antiepileptic drug use for even a few days can raise the risk of seizure and have important medical and social consequences for patients.

These findings offer important take-home messages for physicians, pharmacists, and patients. As Kesselheim explained, "Patients should be aware that their pills may change color and shape, but that even differently-appearing generic drugs are approved by the FDA as being bioequivalent to their brand-name counterparts and are safe to take. Physicians should be aware that changes in pill appearance might explain their patients' non-adherence. Finally, pharmacists should make a point to tell patients about the change in color and shape when they change generic suppliers."

Researchers acknowledge that medication adherence is a multi-faceted issue, but suggest that taking steps to permit (or even require) similarity in pill appearance among bioequivalent brand name and generic drugs may offer a relatively simple way to contribute to better adherence.

This research was supported by a career development award from the Agency for Healthcare Research & Quality (K08HS18465-01), and a Robert Wood Johnson Foundation Investigator Award in Health Policy Research. Dr. Choudhry has received unrestricted research grants from CVS Caremark, Aetna, the Commonwealth Fund, and the Robert Wood Johnson Foundation to study medication adherence. Chicago's next prosecutor to be more of an insider

CHICAGO (AP) ? The lanky, soft-spoken lawman from New York arrived in Chicago with a mandate to clean up corruption-plagued Illinois. And after a decade on the job, Patrick Fitzgerald had helped put two successive governors and a long procession of other public officials behind bars.

Months after the consummate outsider resigned as head of the U.S. attorney's office in Chicago to enter private practice, the White House is expected to name Fitzgerald's replacement soon from among four finalists ? all of whom are comparative Chicago insiders.

Whoever is picked, the next U.S. attorney will step in to what is widely regarded as Chicago's second-most powerful job, next only to the mayor. The chief prosecutor and around 170 assistant attorneys also have an impact beyond Chicago and Illinois, including by handling major terrorism cases.

"The fantastic thing about Fitzgerald was that he maintained his independence," said Kathleen Zellner, a Chicago-based defense attorney. "I'm not saying these candidates won't be independent, but it's hard to decide to prosecute when you have (such close) connections to a town."

The list of four finalists ? Lori Lightfoot, Zachary Fardon, Jonathan Bunge and Gil Soffer ? was recently forwarded to the Obama administration by Illinois' two U.S. senators, who set-up a screening committee to vet a longer list of prospective candidates over several months.

All four know their way around the federal prosecutor's office in Chicago ? one of the nation's busiest ? each having worked there as assistant attorneys at some point. Fardon, for instance, was a member of Fitzgerald's trial team that convicted former Illinois Gov. George Ryan on corruption charges in 2006.

If Lightfoot is named, she would make history as the first African-American and first woman to head the office.

But what stands out about the four, as a group, is that none could be described as an outsider. All four, who are little known outside legal circles, are currently partners in big-name law offices in Chicago. All have spent at least several years of their legal careers in the city.

At the time of his surprise pick in 2001, Fitzgerald was co-chief of the organized crime and terrorism unit for the U.S. attorney's office in the Southern District of New York. The thinking was that he'd be more willing to go after Illinois politicians because he had no ties to them.

It seemed to work, Zellner said. Ryan, a Republican, and former Gov. Rod Blagojevich, a Democrat, are both in prison on corruption convictions after investigations spearheaded by Fitzgerald. He helped send dozens of other city and state officials to prison.

Appointing someone with Chicago ties may convey confidence that Chicago is no longer as corrupt as it was, said Gal Pissetzky, another Chicago attorney. He said it could signal a desire to shift focus away from corruption and on to other persistent Chicago crime, such as drug trafficking or gang-related murders.

"If you want to tackle these issues, it might make sense to have someone from Chicago," he said. "They know the inner workings of Chicago. And law enforcement will be more cooperative when you bring someone from the inside, from Chicago."

The finalists haven't spoken publicly about their candidacy, or about whether they would change in office priorities.

In a letter to the U.S. senators describing interviews with the four, however, the co-chairs of the screen committee wrote that, "All share the belief ? though with slightly differing ordering ? that the primary subject matter concerns of the office should be: 1. Violence and drugs; 2. Public corruption; 3. Financial crimes, and 4. Terrorism."

Lightfoot seems to have especially strong connections to city government, heading the Chicago police Office of Professional Standards between 2002 and 2005. Other candidates have also held administrative posts, including Fardon when he served as the No. 2 in the U.S. attorney's office in Nashville.

"What you see is that this seems to be a selection of people who are more administrators," said Zellner. "It is almost a retreat from a Pat Fitzgerald-type of prosecutor."

That, she added, didn't mean any one of the candidates wouldn't excel.

"You don't get nominated without having really good credentials," she said. "But it is difficult to know what philosophy someone will have until after a year or so. That transition will take time."

Federal investigations can take years before they result in indictments or go to trial, so any shift in direction under new leadership is likely to be incremental and happen over years.

A change in style is more likely, said Pissetzky.

As he racked up flashy convictions ? including of reputed mobsters and terrorists ? Fitzgerald gained a reputation as a no-nonsense prosecutor who erred on the side of secrecy and typically eschewed banter with reporters. He could be tenacious to a fault, defense attorneys said. Over the years, many complained that Fitzgerald pursued their clients with too much fervor, loading indictments up with as many charges as he could muster.

It's a style that his successor won't necessarily emulate.

Said Pissetzky, "I think they will try to make their own mark rather than trying to follow in his footsteps."

It shouldn?t be a primary source of your financing. Its primary use should be in establishing a healthy picture for your banker, so that you can invest his money instead.

Stocks & Bonds

You might not have an investment portfolio, but if your current job offers stocks, it?s time to evaluate what you have or start buying in while you?re still planning your store. Depending on how you plan to finance your store, you might want to keep the stocks or liquidate your holdings. If you do cash out, watch your timing and try to wait as long as possible; if you decide not to enter the game retail industry after all, you probably can?t buy back into those stocks.

Trade Magazines

Virtually every industry has its own trade magazines, and these trades pay good money for articles. If you can explain something in simple terms, stick to a topic, and follow a publisher?s guidelines, you might be able to crank out an article or two. With some trades paying up to $2 a word, a single article might net $500 to $6,000. You don?t have to be a professional writer, either. The most important skill is knowledge of your topic.

Coin Collecting

I used to deliver pizzas. At the end of the night, instead of turning in my coins with my other money, I threw them in a jar. Every Christmas season, my wife and I rolled that change to pay for the holidays. The last year we did that, we cashed in $1087. Corral all of these stray dollars into one place and you might be surprised at how much it is.

Turn a bunch of junk into cash. A successful yard sale might net up to $1,000, depending on how much you have to sell. Just as nice, it might clear up space in your garage where you can stockpile fixtures until you?re ready to move into your storefront.

Your Test-Market

In an earlier article, I recommended testing your local market by running a convention space, hitting trade shows, or helping an existing retailer. I?ve even seen game retailers run flea market tables. This method works best with second-hand merchandise that you can buy cheaply and sell quickly, often starting with a personal collection. Start with just $200 or so in inventory and build up from there, reinvesting your sales into more merchandise. If you?re lucky, you might start with $2,000 to $5,000 in second-hand goods that you won?t have to buy when you open your store.

Demo Materials

If you took my advice to become a demo volunteer, you might have a collection of materials to start with. If you really ran with it and become a volunteer for multiple companies, you could have more materials than some stores use. Between promo material and any unopened compensation they sent you in the form of product, you could have a few hundred dollars in assets.

Start a Fixture Collection

Retailers often end up becoming fixture junkies. You can?t help but look around any given store, not just to see what they have, but how they display it. You?ll ask yourself ?Can I use something like that?? surprisingly often. Start your neurosis early by acquiring cheap and free fixtures while you?re still in the planning stage, months before you sign a lease. Picking up a usable piece each month might save hundreds or thousands off of your startup costs by the time you open.

You can find cheap fixtures in auctions and going-out-of-business sales around town. You might make an offer to a landlord who?s stuck with the property of a vanished tenant. You can find free fixtures occasionally when stores of any size?convenience to big-box?remodel. Manufacturer Charity

The manufacturers whose products you?ll sell have a vested interest in seeing you succeed. The attitude of manufacturers toward retailers runs the gamut from heavy favor to outright disdain, but asking for promotional materials and products can yield some much-needed inventory. As always, don?t ask for things you don?t need and don?t get greedy. Asking for a bonus on top of an order is better than just asking for a freebie outright, for example.

Buy Debt

While many people avoid debt, the right use of your company?s credit and your personal credit is a tool for you to use. Carpenters don?t refuse to use nails out of some crazy sense of elitism or personal acumen. Neither should you be afraid to indebt your company if it?s the best thing for your company?s overall health.

Assume the Seller?s Debt

If you plan to start by buying an existing business, then you can reduce your purchase price by acquiring debt at the same time. Suppose, for example, that a store owner wants $25,000 for his store. When you ask about his current debts, he tells you that he owes his distributors $4,000 in current and late bills. Tell him you?d be willing to take over that debt if he?ll reduce his asking price by $4,000. If he has any intention at all of paying that bill, there?s little reason to reject your offer. You can then negotiate repayment terms with the distributors to whom he owes money. Offering to repay $200 on top of each weekly order defers that final payment by 5 months.

Continue that exercise with each of the seller?s individual outstanding debts (rent, utilities, other service providers, etc.) and you might save yourself $15,000 in upfront cash. You?ll pay off most or all of it eventually, but you?ll reduce the startup funds needed by a substantial amount.

Credit Cards

No sane person would tell you to put all of your startup expenses on a credit card, but it can make sense to finance your capital reserve on personal credit of one kind or another. Notice the careful and deliberate use of the word ?can.? That doesn?t mean it always makes sense. Some factors in favor of this plan include a) a low burn amount needed, b) low interest rates on the credit cards , c) a strong incentive plan with your credit card company, or d) high local commercial loans rates.

Bring on Investors

Someone investing a few thousand dollars can put you over a benchmark and turn on the light green for you to continue forward. These investments need not be large. An investment of $500 up front could be worth far more than its face value in its reduction of your primary loan repayment, depending on how you word your investor agreements (hint: defer any repayment for six months to a year after opening). Such low-dollar investments are much easier to get from friends and family than a $20,000 lump sum.
